If you submit the contact form, the information you provide, including your name, email address, subject, and message, is used to respond to your inquiry. Messages are sent by email to the website administrator’s configured address. The form does not create a separate message archive in the WordPress database, although copies may be retained in the recipient’s email system or by configured mail services.
Do not submit medical records, patient identifiers, payment details, or other sensitive health information. This contact form is for general website inquiries. It is not a patient portal, appointment system, or monitored emergency service.
Technical operation
The hosting provider and security services may process standard technical request information. To limit repeated form submissions, this site temporarily stores a one-way hash derived from the submitting connection’s IP address. This rate-limit entry expires after ten minutes. The form also uses a security token and a hidden field to help reduce automated submissions.
This package does not add analytics, advertising trackers, or third-party embeds. WordPress or other installed services may use cookies or process technical data according to their own configuration. External links take you to independently operated websites with separate privacy practices.
